PURPOSE OF JOB:  The School-Home Coordinator works as the school connection for between the school and the home to help improve student attendance and insure academic success. The School-Home Coordinator works with students whose ability to function in the school environment has been affected by issues such as housing instability, truancy, poor attendance, substance abuse, and/or behavioral problems and deals immediately with the individual student problems in a constructive, supportive, non-judgmental manner. The School-Home Coordinator helps increase parent participation in school activities and encourages positive communication between home and school thus enhancing student achievement and attendance as defined by Federal Guidelines. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:  include the following. Other duties may be assigned.

 

  1. Establishes, utilizes and maintains an effective system for working with students, parents, schools, social agencies and other individuals and agencies needed to assist in resolving attendance issues and problems to increase student attendance and achievement
  2. Creates and maintains accurate and adequate files regarding attendance, student progress and follow-up information, reports, records of contacts, referrals, action plans, assessments for each student record
  3. Assists students, families and school personnel in developing agreed upon documented goals and outcomes; monitors progress and communicates with appropriate individuals and agencies
  4. Provides information to school personnel to increase awareness and understanding of home and family conditions that may affect the child progress in school
  5. Serves as a major communication link between youth and school
  6. Perform needs assessment on all student referrals
  7. Maintain a log of services provided to students and parents, including: student progress notes, logs, contact notes, referrals, follow-up, reports, retention, etc.
  8. Assist school in developing plans for parent participation with ongoing communication between home and school.
  9. Work cooperatively with parents and other classroom volunteers.
  10. Assist administration in providing information to students, parents and community on family support services provided through the District to improve student academic achievement and school performance. This includes and not limited to participation in health resources under the Federal Medicaid program.
  11. Provide educational information to school, students, parents and community on family support services provided through the District to improve student academic achievement and school performance.
  12. Involve parents in the planning and implementation of the school parental involvement plan and school wide Title I Plan.
  13. Assist in the planning of monthly workshops and information for parents on the district Monthly ThemesĀ and Title I Mandates.
  14. Prepare extended learning packets for parents to utilize at home with their children.
  15. Assist school and Parent Resource Center Coordinator in planning, scheduling, and implementation of special department initiatives in order to meet objectives and schedules.
  16. Assist school in developing a sustained, systematic plan for parent participation with on-going communication between school and home.
  17. Coordinate and integrate parent involvement strategies with other programs.
  18. Assist with the school newsletter.
  19. Coordinate with the school SAC to set up parent volunteer programs.
  20. Attend and participate in scheduled professional development.
  21. Complete, review, and edit required program reports and documents.
  22. Provide monthly status reports.
  23. Attend monthly Title I meetings and other meetings as assigned.

 

SUPERVISORY RESPONSIBILITIES:  none

REQUIRED QUALIFICATIONS:

B.A. or B.S. in Social Work, Counseling, Education, Health Care, Substance Abuse, Sociology, or Psychology. (Two years related experience and/or training may be substituted for each year of college)

Experience working with culturally and ethnically diverse populations and at-risk students

Experience working with and accessing various individuals, social service agencies and community agencies to access social services

Experience demonstrating office and computer skills

Experience conducting concise and meaningful assessments, formulating action plans, writing progress reports and summaries, communicating with students and individuals needed to assist

TERMS OF EMPLOYMENT:

Length of work year: 205 days; 10.0/Months; Grant funded position
